VAT on Dental Treatment in the UAE: When You Pay 5% and When You Don't
UAE dental services are largely VAT-exempt — but not always. The line between medical and cosmetic is where the tax lives.
Read this for a UAE city:
The UAE Federal Tax Authority classifies healthcare services as VAT-exempt when they qualify as preventive or curative. Cosmetic-only procedures are taxable at the standard 5% rate.
### VAT-exempt (0% to patient)
- Extractions (medical or wisdom)
- Fillings (composite or amalgam)
- Root canal treatment
- Crowns and bridges where medically indicated
- Dentures
- Periodontal treatment
- Emergency dentistry
- Pediatric preventive care (varnish, sealants)
- Orthodontics with medical justification (severe malocclusion, sleep apnoea, cleft palate)
### VAT-taxable at 5%
- Teeth whitening (unless post-medical treatment)
- Cosmetic veneers with no functional indication
- Cosmetic gingivectomy / gum contouring
- Diamond/gem tooth attachments
- Orthodontics for pure aesthetics (mild crowding without functional concern)
### The grey area
Whether Invisalign or veneers attract VAT depends on the treating clinician's clinical documentation. A clinic classifying every Invisalign as ‘cosmetic’ is not necessarily wrong — but a well-documented crossbite or airway justification means no VAT. This is worth asking about; on a AED 25,000 case, VAT is AED 1,250.
### On your invoice
UAE clinics must itemise VAT separately. A single line saying ‘veneers with VAT included’ is not compliant; you can request a compliant tax invoice showing the exempt vs taxable portion.
### Insurance interaction
Insurance covers the pre-VAT amount for medically necessary work. If you're paying out-of-pocket for a hybrid case (medical crown + cosmetic whitening), only the whitening portion is taxed.
### For non-residents (dental tourists)
Dentistry qualifies for the UAE ‘Tax Refund for Tourists’ scheme only for VAT-taxable purchases > AED 250. Purely medical work isn't taxed, so nothing to refund. Cosmetic work purchased by tourists can be refunded at the airport tax-refund kiosk before departure.
### Bottom line
Ask the clinic to specify: is this treatment classified as medical or cosmetic for VAT? On any bill over AED 2,000 the answer matters.
