Baby-Bottle Tooth Decay: The Silent Emergency in UAE Nurseries
Nursery caries is the single most common reason UAE children under five need general anaesthesia. It is entirely preventable.
Read this for a UAE city:
Also called Early Childhood Caries (ECC), baby-bottle tooth decay occurs when milk, formula or juice pools around the upper front teeth during nighttime feeds. Sugars ferment for hours; enamel dissolves in weeks.
### Why the UAE sees more of it
- **Late weaning**: cultural comfort with bottle-to-sleep well past 18 months.
- **Sweetened labaneh, laban and yoghurt drinks** served at bedtime.
- **Grandparent care** during working parents' hours — sugar as affection.
### The three stages
1. **White-spot lesion** (reversible with fluoride varnish, ~AED 200)
2. **Cavity in enamel** (needs a filling, ~AED 400–700)
3. **Pulp involvement** (needs pulpotomy or extraction, ~AED 900–2,500)
By stage three, most children cannot tolerate treatment awake. Full-mouth rehabilitation under general anaesthesia at a JCI-accredited hospital runs AED 12,000–22,000 in Dubai — and 70% is typically covered by comprehensive insurance.
### Prevention in one paragraph
By the child's first birthday: bottle only for meals, never for sleep. Wipe gums after night feeds until teeth erupt, then brush twice daily with a smear of 1,000-ppm fluoride toothpaste. Water in the sippy cup, not juice.
### If it's already started
Early-stage lesions arrest with silver diamine fluoride (SDF, AED 300–450 per tooth) — it turns the spot black but stops decay in 80% of cases and avoids anaesthesia for toddlers.
